This has been reported by multiple users, and I've experienced it myself. It occurs for both Firefox and IE browsers. Since the site uses templates extensively, this is causing real problems cleaning up afterwards.

On the "Add a new page" dialog, if the "Add Page" button is clicked quickly after a template is selected, then when the new page is created, it does not have the template applied. Do you find this valuable?
