Continuous password-reseting problem

One of my wiki members keeps having a recurring problem, where she tries to log in to our wiki, but is instructed to reset her password repeatedly, and that is the only way she can get back into the site, by doing it each time. Why is this happening, and what is the solution?

Towards the middle of the page, it describes the problem of having to reset the password every time one signs in. That is the only knowledge I can offer to you.

This usually means the user is entering the wrong username when they try to sign in. Do you find this valuable?
