Creating a Form and Finding the Code

1. Go to www.formsmarts.com, and register.
2. Scroll down the page. In the bottom left corner, click on "My Forms".
3. Fill in your form's information, and click on "Create Form".
4. You will then see that the form you created has been transferred to the box above. Now, click on "Add Fields" and add your form's fields.
5. After you're satisfied with your fields, click on the "Forms" button beside the "Fields" button.
6. Copy the Embed Code of your Form.

Adding the Form to your site

1. To add your widget to a page, click on the "EasyEdit" button and the toolbar will appear.
   
   
2. Place your cursor where you'd like to display your widget, and click on the "Widget" button in the toolbar.
   
   
   
   The "Add Widget" dialog will appear.
   
   
   
3. Select "Other Widget."
   
4. The "Add Widget" dialog will appear. Paste your widget export code into the open text field and then click "Add Widget."
5. After you've added your widget, an "Edit Widget" dialog will appear. You can change the size of your widget by using the (-) or (+) options. You can also change where your widget appears in relationship to the copy on the page by using the layout icons. When you're satisfied with the size and layout of your widget image, click on the "Done" button.
